Case
Management
Oftentimes, returning back to life after release may come with challenges. SACA case managers provide individualised case management and counselling services to clients and their families, with the intention of identifying core rehabilitation needs and providing support to help break the cycle of re-offending.
WHAT IS
CASE MANAGEMENT?
Case Management is a collaborative process where a professional case worker assesses the needs of the client (and family), and where appropriate, arranges, coordinates, monitors, evaluates, and advocates for a range of services, from counselling support to information and referral services, to meet the identified needs of the client.
